Potassium citrate malate is used to support or treat gout based on scientific rationale rather than tradition. Gout is caused by deposition of monosodium urate crystals in joints, resulting from elevated serum uric acid levels. Potassium citrate, and by extension potassium citrate malate, acts as an alkalinizing agent, raising urinary pH. This increased alkalinity enhances uric acid solubility and promotes its excretion, potentially reducing uric acid crystal formation and deposition.
Several studies have shown that alkalinization of urine with potassium citrate can decrease the risk of uric acid stone formation and lower uric acid levels in the urine, which is relevant to gout management. For example, a study published in the Journal of Urology (2010) demonstrated that potassium citrate supplementation increased urinary pH and reduced uric acid supersaturation in patients with a history of uric acid nephrolithiasis. While most evidence focuses on uric acid kidney stones, the underlying mechanism—reducing uric acid precipitation—also applies to gout, though direct clinical trials in gout patients are limited.
Guidelines from organizations such as the American College of Rheumatology primarily recommend urate-lowering therapies (e.g., allopurinol) for gout, but potassium citrate is acknowledged as a supportive measure for uric acid stone prevention, which often coexists with gout. Thus, the evidence for potassium citrate malate’s use in gout is moderate and mechanistically plausible, but it is not a primary therapy and should be considered adjunctive to established treatments.
