Earthworm (commonly referred to in Traditional Chinese Medicine as "Di Long") has been used in various traditional medicine systems, particularly in China, for a range of ailments including convulsions, hypertension, and inflammatory conditions. Its use for gout is primarily justified by traditional practices rather than robust scientific studies. In TCM, earthworm extracts are sometimes prescribed for their purported ability to "clear heat" and "remove obstruction from the channels," which are metaphorically linked to symptoms of gout, such as joint swelling and pain. Some animal studies have evaluated anti-inflammatory and uric acid-lowering effects of earthworm extracts, but these are limited in quality and quantity, and results have not been consistently replicated or confirmed in human clinical trials. There is currently a lack of high-quality, peer-reviewed evidence directly supporting the efficacy or safety of earthworm preparations for treating or managing gout in humans. Thus, while its use for gout persists in traditional contexts, it is not considered evidence-based by modern scientific standards.
