Beta-hydroxybutyrate (BHB) has been investigated for its potential to mitigate free radical damage, largely due to its role as a ketone body produced during periods of fasting, prolonged exercise, or ketogenic diets. Scientific studies, primarily preclinical and some early human research, suggest that BHB may exert antioxidant effects through several mechanisms. BHB has been shown to enhance the expression of antioxidant genes via activation of the Nrf2 pathway, reduce oxidative stress by inhibiting histone deacetylases (HDACs), and decrease the production of reactive oxygen species (ROS) in mitochondria. Animal studies demonstrate that exogenous BHB supplementation can reduce markers of oxidative stress and neuronal damage in models of neurodegenerative disease and brain injury. However, while mechanistic and animal studies are promising, clinical evidence in humans remains limited and largely preliminary. Some small trials and observational studies suggest a potential reduction in oxidative stress markers in individuals on ketogenic diets or those receiving exogenous ketones, but robust, large-scale, controlled trials are lacking. As such, while there is a biological rationale and some scientific support for BHB's role in combating free radical damage, the clinical relevance and magnitude of benefit for this specific application in humans are not yet firmly established.
