Alpha-Ketoglutarate (AKG) has been investigated for its potential antioxidant properties and its role in mitigating free radical damage. AKG is a key intermediate in the Krebs cycle and plays a role in cellular energy metabolism. Several preclinical studies suggest that AKG can scavenge reactive oxygen species (ROS) and may help reduce oxidative stress in cells. For example, some animal studies have demonstrated that AKG supplementation can decrease markers of oxidative damage and improve antioxidant enzyme activity. Additionally, AKG can act indirectly by supporting glutamine and glutathione synthesis, both of which are important for cellular redox balance.

However, human clinical evidence is limited. Most of the available data come from in vitro or animal models, with few robust trials in humans. Some studies in critical care settings have used AKG as part of amino acid supplementation, noting potential improvements in antioxidant status, but these do not conclusively establish AKG as an effective treatment for free radical damage in general populations. As such, while there is a plausible scientific rationale and some supportive preclinical evidence, the overall evidence base remains modest, and further research—particularly well-designed human trials—is needed to substantiate its efficacy for this indication.
