Specialized pro-resolving mediators (SPMs), such as resolvins, protectins, and maresins, are endogenous lipid mediators derived from omega-3 fatty acids that play critical roles in actively resolving inflammation. There is a growing body of preclinical (animal and cell-based) research suggesting that SPMs may help limit or reverse fibrosis—the pathological accumulation of connective tissue associated with chronic inflammation in organs such as the liver, lungs, kidneys, and heart. Mechanistically, SPMs have been shown to reduce the recruitment and activation of pro-fibrotic immune cells, downregulate the production of fibrogenic cytokines (such as TGF-β), and promote the clearance of apoptotic cells and extracellular matrix components, all of which are key processes in the resolution of fibrosis.

However, while these findings are promising, most of the evidence to date comes from animal models or in vitro experiments. For instance, studies in murine models have demonstrated that administration of SPMs can attenuate lung and liver fibrosis by modulating inflammatory pathways and fibroblast activity. As of now, there are few, if any, robust clinical trials in humans directly assessing the efficacy of SPM supplementation in treating established fibrosis. Hence, the evidence is preliminary and rated as 2 out of 5—suggesting some scientific basis and promising preclinical data, but a lack of strong human clinical validation. More clinical research is needed to confirm safety, efficacy, and optimal dosing in humans with fibrotic diseases.
