Vitamin D has been investigated for its potential role in supporting or treating Fibromyalgia Syndrome (FMS), primarily because vitamin D deficiency is common in the general population and some symptoms of deficiency—such as musculoskeletal pain, fatigue, and mood disorders—overlap with those of fibromyalgia. Observational studies have reported a higher prevalence of vitamin D deficiency in patients with FMS compared to controls. Some small interventional trials suggest that vitamin D supplementation may lead to modest improvements in pain and quality of life in vitamin D-deficient fibromyalgia patients. For example, a 2014 randomized controlled trial (Rashid et al., Pain Medicine) found that vitamin D supplementation reduced pain scores in deficient individuals with FMS. However, other trials have failed to find significant benefits, and systematic reviews have generally concluded that the evidence is weak and inconsistent. Current clinical guidelines do not recommend vitamin D supplementation specifically for FMS unless a deficiency is documented.

In summary, while there is some scientific rationale and preliminary evidence supporting the use of vitamin D in FMS—particularly in those who are deficient—the overall quality of evidence is low. The benefit appears limited to correction of deficiency rather than a direct treatment effect on fibromyalgia itself. Therefore, vitamin D is not considered a first-line therapy for FMS, but screening for and correcting deficiency may be reasonable as part of comprehensive care.
