Ubiquinol, the reduced and active form of coenzyme Q10 (CoQ10), has been investigated as a supportive treatment for Fibromyalgia Syndrome (FMS) due to its roles in mitochondrial energy production and as an antioxidant. Scientific interest in CoQ10/ubiquinol for FMS is based on findings that patients with fibromyalgia often have mitochondrial dysfunction, increased oxidative stress, and low levels of CoQ10. Several small clinical trials and pilot studies have explored the effects of CoQ10 supplementation (in both ubiquinone and ubiquinol forms) on fibromyalgia symptoms. For example, a randomized controlled trial published in 2013 (Cordero et al., Nutrition) found that 300 mg/day of CoQ10 for 40 days significantly reduced pain, fatigue, and morning tiredness in FMS patients compared to placebo. Other studies have reported improvements in clinical symptoms, oxidative stress markers, and mitochondrial function.

However, most studies have small sample sizes and short durations, and many use CoQ10 (ubiquinone) rather than ubiquinol specifically. While the theoretical rationale is strong and preliminary results are promising, the overall quality and quantity of evidence are limited, and more large-scale, high-quality trials are needed. As such, the use of ubiquinol for fibromyalgia is supported by early scientific research, but evidence remains preliminary, justifying a moderate-low evidence rating.
