D-Ribose is a naturally occurring sugar involved in the production of adenosine triphosphate (ATP), the primary energy carrier in cells. Its use in supporting or treating Fibromyalgia Syndrome (FMS) is primarily based on its role in cellular energy metabolism, under the hypothesis that impaired energy metabolism may contribute to FMS symptoms such as fatigue and muscle pain. There is some limited scientific evidence supporting its use: a small open-label pilot study by Teitelbaum et al. (2006) involving 41 patients with fibromyalgia and/or chronic fatigue syndrome reported improvements in energy, sleep, mental clarity, pain, and well-being after D-Ribose supplementation (5 g, three times daily for up to 3 weeks). However, this study lacked a placebo control and had a small sample size, raising concerns about bias and generalizability. No large-scale, randomized, placebo-controlled trials have been published to date that clearly demonstrate efficacy or safety for D-Ribose in FMS. Systematic reviews and clinical guidelines do not currently recommend D-Ribose as a standard treatment for fibromyalgia, citing insufficient evidence. Thus, while there is some preliminary scientific investigation, the overall quality and quantity of evidence are low, and further research is needed to establish its effectiveness and safety.
