Vitamin E has been studied for its potential role in managing nonalcoholic fatty liver disease (NAFLD), primarily due to its antioxidant properties. Most clinical research and guidelines have focused on alpha-tocopherol, the most common and biologically active form of Vitamin E in supplements and therapy. Beta-tocotrienol is a less common form of Vitamin E, and while tocotrienols as a group have demonstrated antioxidant and anti-inflammatory effects in preclinical studies, there is limited direct clinical evidence specifically for beta-tocotrienol in fatty liver disease. Several randomized controlled trials and meta-analyses have shown that Vitamin E (mainly alpha-tocopherol) can improve liver histology and biochemical markers in nondiabetic adults with NAFLD or nonalcoholic steatohepatitis (NASH). Major liver societies, such as the American Association for the Study of Liver Diseases (AASLD), recommend considering Vitamin E (alpha-tocopherol) for nondiabetic patients with biopsy-proven NASH. However, clinical data on tocotrienols, including beta-tocotrienol, are sparse. Some animal and cell studies suggest tocotrienols may reduce hepatic lipid accumulation and inflammation, but robust human data are lacking. In summary, while there is scientific rationale and some evidence for Vitamin E’s role in fatty liver disease, this is mostly limited to alpha-tocopherol. The evidence specifically for beta-tocotrienol is preliminary and not sufficient to support strong clinical recommendations.
