Phospholipids, such as phosphatidylcholine, have some scientific rationale for use in supporting or treating fatty liver disease (hepatic steatosis), particularly non-alcoholic fatty liver disease (NAFLD). Phospholipids are essential components of cell membranes and play a role in lipid metabolism and transport. Some studies, primarily from Eastern Europe and Asia, have examined the use of "essential phospholipids" (EPLs), a mixture usually derived from soybeans, in the management of fatty liver conditions. These studies suggest that phospholipids may help to repair damaged liver cell membranes, improve liver enzyme levels, and potentially reduce liver fat accumulation.

However, the overall quality of evidence is modest. Some randomized controlled trials and meta-analyses report mild improvements in biochemical markers (such as ALT and AST) and imaging findings, but the clinical significance and long-term benefits remain uncertain. The beneficial effects are often observed in studies sponsored by manufacturers, and there are methodological limitations such as small sample sizes, short durations, and lack of hard clinical endpoints (like progression to cirrhosis or liver-related mortality). International guidelines do not currently recommend phospholipids as a standard therapy for fatty liver disease.
