Job's Tears (Coix lacryma-jobi), also known as adlay or Chinese pearl barley, is traditionally used in East Asian medicine for various health conditions, including liver disorders. Recent scientific studies have investigated its potential effects on fatty liver disease (hepatic steatosis), particularly non-alcoholic fatty liver disease (NAFLD). Preclinical studies in animal models have shown that extracts of Job's Tears may help reduce hepatic lipid accumulation, decrease oxidative stress, and modulate inflammatory pathways, all of which are relevant to fatty liver disease pathology. Some in vitro and animal data suggest improvements in liver enzyme levels, lipid profiles, and markers of liver inflammation after administration of Job's Tears extracts.
However, the current evidence is limited primarily to laboratory and animal studies, with very few, if any, high-quality human clinical trials directly assessing its efficacy for fatty liver disease. The proposed mechanisms include antioxidant, anti-inflammatory, and lipid-lowering effects, possibly mediated by polyphenols and coixol found in the plant. While these findings are promising, there is insufficient clinical data to make firm recommendations for its use in treating fatty liver disease in humans. Thus, the evidence level is rated as 2, reflecting preliminary scientific support but a lack of robust clinical validation.
