Glycyrrhizinic acid, a major active component of licorice root (Glycyrrhiza glabra), has been investigated for its potential hepatoprotective effects, including in the context of fatty liver disease (non-alcoholic fatty liver disease, NAFLD). Several preclinical studies in animal models indicate that glycyrrhizinic acid may reduce hepatic steatosis, inflammation, and fibrosis. The proposed mechanisms include its antioxidant properties, inhibition of lipid peroxidation, and modulation of pro-inflammatory cytokines. In addition, glycyrrhizinic acid appears to inhibit the activation of hepatic stellate cells, which play a key role in liver fibrosis.
There are also a limited number of clinical studies, primarily from East Asia, where intravenous glycyrrhizinic acid preparations have been used for chronic liver disease. Some small trials and observational studies suggest improvements in liver enzymes (such as ALT and AST) in patients with fatty liver disease or chronic hepatitis. However, the quality of clinical evidence is generally low, with small sample sizes, short durations, and lack of rigorous controls. Major guidelines do not recommend glycyrrhizinic acid as a standard treatment for NAFLD, and its use remains largely complementary or investigational. Adverse effects such as pseudoaldosteronism and hypertension are also concerns with prolonged use. In summary, there is some scientific rationale and preliminary evidence supporting glycyrrhizinic acid for fatty liver, but robust clinical validation is lacking.
