Fructo-oligosaccharides (FOS) are prebiotic fibers that have been studied for their effects on gut microbiota and metabolic health, including potential benefits for non-alcoholic fatty liver disease (NAFLD). Several animal studies suggest that FOS supplementation can modify gut microbiota composition, increase production of beneficial short-chain fatty acids (SCFAs), and reduce hepatic lipid accumulation. For example, rodent models of fatty liver disease have shown that FOS can decrease liver fat content and improve markers of liver injury, likely through modulation of inflammation and improvement in insulin sensitivity (Zhu et al., 2015). However, evidence in humans is limited and largely preliminary. A few small clinical trials have reported improvements in surrogate markers related to liver health, but consistent reductions in liver fat or robust improvements in liver function have not been conclusively demonstrated (Daubioul et al., 2005). The proposed mechanisms involve the modulation of gut-liver axis signaling, reduction of endotoxemia, and improvement in metabolic parameters, but these remain to be validated in larger, well-controlled human studies. Overall, while there is some scientific rationale and preclinical evidence supporting the use of FOS for fatty liver disease, high-quality clinical evidence is still lacking. Thus, FOS cannot yet be considered a proven therapy for fatty liver disease in humans.
