Choline is an essential nutrient involved in liver function, lipid metabolism, and the transport of fats. Its role in the management of Fatty Liver Disease (particularly nonalcoholic fatty liver disease, NAFLD) has scientific backing, though the strength of evidence is moderate and not yet conclusive. Choline deficiency is known to cause liver dysfunction and fat accumulation in the liver (hepatic steatosis) in both humans and animal studies. Several clinical studies and controlled trials have shown that low dietary choline intake can induce or worsen fatty liver, while supplementation can help reverse or prevent these changes, especially in individuals with genetic variations affecting choline metabolism or during periods of increased need (like pregnancy).

The Institute of Medicine recognizes that inadequate choline intake can result in liver damage. Moreover, a few studies in patients with NAFLD or nonalcoholic steatohepatitis (NASH) have reported improved liver function markers and reduced hepatic fat with increased choline intake. However, large-scale clinical trials evaluating choline supplementation specifically as a therapy for NAFLD are limited. Current guidelines do not universally recommend choline supplementation, but it is considered in cases of deficiency or in populations at risk.

In summary, while there is a clear scientific rationale and some supporting evidence, more robust clinical data are needed to establish choline as a standard treatment for fatty liver disease. Its use is justified when deficiency is present, and it remains an active area of research for broader therapeutic applications.
