GW501516 (also known as Cardarine) is a synthetic Peroxisome Proliferator-Activated Receptor delta (PPARδ) agonist. It has been studied primarily for its effects on lipid and glucose metabolism. Animal studies and early-phase human trials have shown that GW501516 activates PPARδ, which leads to enhanced fatty acid oxidation, increased HDL ("good") cholesterol, and decreased triglycerides. Rodent studies demonstrated that administration of GW501516 significantly increased the body's ability to utilize fat as an energy source, reducing fat mass and improving metabolic parameters. In small human trials, GW501516 increased HDL cholesterol and lowered triglycerides, and some data suggest improved markers of metabolic syndrome and insulin sensitivity. However, the evidence base is still limited, and use in humans is not approved due to significant safety concerns, including carcinogenicity observed in animal studies.

While the molecular and physiological rationale for its effect on fat metabolism is strong, and some supportive animal and early human data exist, there are no large-scale, long-term clinical trials validating its efficacy or safety in treating poor fat metabolism in humans. Its use is banned in sports and discouraged in medicine outside of research settings. Thus, the evidence is best rated as moderate (3/5): promising mechanistic and preclinical data, but insufficient high-quality human outcomes data due to safety issues.
