27-deoxyactein is a triterpene glycoside found primarily in the roots of Actaea racemosa (black cohosh), a plant traditionally used in herbal medicine to support women's health, especially during menopause. The traditional use of black cohosh, and by extension its constituent 27-deoxyactein, is to alleviate symptoms associated with hormonal changes, such as hot flashes, mood swings, and other menopausal complaints. Historical records indicate that Native American communities utilized black cohosh for various gynecological conditions long before its introduction into Western herbal practices.

While black cohosh as a whole has been the subject of numerous clinical trials and reviews, isolating the effects of 27-deoxyactein itself is more challenging. Most studies focus on the crude extract or standardized preparations, rather than individual constituents. Despite this, 27-deoxyactein is often cited as a marker compound for black cohosh supplements. There is some evidence that black cohosh extracts may modulate serotonergic pathways and possibly have mild estrogenic activity, but the role of 27-deoxyactein specifically remains unclear. As such, the traditional use of black cohosh (and thus 27-deoxyactein-containing preparations) to support the estrogen body system is established, but scientific validation for this specific constituent is limited, warranting a modest evidence rating.
