Eurycoma longifolia (commonly known as Tongkat Ali) has a long history of traditional use in Southeast Asia as an aphrodisiac and remedy for male sexual dysfunction, including erectile dysfunction (ED). Its traditional reputation has prompted scientific investigation, and several preclinical and clinical studies have explored its effects on male sexual health. The available evidence suggests that Eurycoma longifolia may have a modest positive effect on erectile function, possibly through mechanisms involving increased testosterone production, enhanced libido, and improved sperm quality. Some small randomized controlled trials and observational studies have reported improvements in erectile function scores and sexual satisfaction in men taking Eurycoma longifolia extracts versus placebo. However, these studies are often limited by small sample sizes, short durations, or methodological weaknesses. Systematic reviews and meta-analyses have generally concluded that while preliminary results are promising, the quality and size of the evidence base do not yet allow for strong recommendations. Larger, better-designed human trials are needed to definitively establish efficacy and safety. Thus, while there is some scientific validation for its use in supporting ED, the evidence is not robust, and its use should be considered with caution.
