Arginine is an amino acid that serves as a precursor to nitric oxide (NO), a molecule that plays a crucial role in vasodilation and blood flow regulation. Scientific interest in arginine for erectile dysfunction (ED) stems from the fact that NO-mediated vasodilation is essential for achieving and maintaining erections. Several clinical studies and systematic reviews have explored the efficacy of oral arginine supplementation in men with ED. Some randomized controlled trials (RCTs) have shown modest improvements in erectile function, particularly in cases of mild to moderate ED, and especially when arginine is combined with other agents like pycnogenol. However, results are inconsistent: some studies report no significant benefit over placebo, and the effect sizes, where present, are generally small.

The 2017 American Urological Association guidelines and systematic reviews note that while arginine may have a physiological rationale and some supporting evidence, the overall quality and quantity of evidence is limited, and arginine is not considered a first-line or well-established therapy for ED. Side effects are generally mild, but its safety and efficacy in long-term use have not been thoroughly established.
