Vitamin B6 (pyridoxine) has a specific, scientifically validated role in the management of certain rare forms of epilepsy, particularly pyridoxine-dependent epilepsy (PDE). PDE is a rare genetic disorder, typically presenting in neonates or infants, in which a deficiency of the enzyme antiquitin leads to the accumulation of compounds that interfere with the function of pyridoxal phosphate, the active form of vitamin B6. This disruption impairs the synthesis of neurotransmitters such as gamma-aminobutyric acid (GABA), which is inhibitory in the central nervous system and plays a key role in controlling neuronal excitability. In patients with PDE, seizures are refractory to conventional antiepileptic drugs but respond dramatically to pharmacological doses of vitamin B6. The diagnosis is confirmed by seizure cessation after intravenous or oral administration of pyridoxine. However, outside these rare, specific metabolic epilepsies, there is little evidence supporting the use of vitamin B6 supplementation to treat or prevent epilepsy in the general population. Clinical trials have not shown benefit in most other epileptic disorders, and unnecessary supplementation can cause toxicity. In summary, the scientific evidence strongly supports the use of vitamin B6 for rare, inborn errors of metabolism that lead to specific vitamin B6-dependent epilepsies, but not as a broad treatment for epilepsy in general.
