Ketone salts are used as a supplemental means of elevating blood ketone levels, mimicking the metabolic state induced by a ketogenic diet. The ketogenic diet, which promotes the production of endogenous ketones (mainly beta-hydroxybutyrate), has a well-established scientific basis for the management of epilepsy, particularly in drug-resistant cases and in pediatric populations. The anti-epileptic effects are thought to be due to alterations in neuronal energy metabolism, neurotransmitter balance, and potential neuroprotective effects of ketones.

However, the evidence for exogenous ketone salts themselves (as opposed to the ketogenic diet) in epilepsy is limited. Preclinical animal studies suggest that exogenous ketones can reduce seizure frequency and severity, but robust human clinical trials are lacking. Some small pilot studies and case reports indicate that ketone supplementation may help increase seizure threshold, but these findings are preliminary and not yet widely replicated in controlled settings.
