Specialized pro-resolving mediators (SPMs) are bioactive lipid compounds derived from omega-3 and omega-6 fatty acids that play a critical role in actively resolving inflammation. The rationale for their use in endometriosis stems from the fact that endometriosis is characterized by chronic, unresolved inflammation within the pelvic cavity. Preclinical studies, particularly in animal models, have shown that SPMs such as resolvins, protectins, and maresins can reduce inflammatory signaling, decrease lesion size, and limit pain-like behavior associated with endometriosis-like lesions. These effects appear to be mediated by modulation of immune cell infiltration, reduction in pro-inflammatory cytokine expression, and promotion of tissue repair.

However, direct clinical evidence in humans remains limited. Most available data are derived from laboratory and animal studies rather than randomized controlled trials in women with endometriosis. Some early-phase human studies and case reports suggest potential benefits, but robust, high-quality clinical trials are lacking. As such, while the underlying mechanism is biologically plausible and supported by preclinical research, the clinical efficacy of SPM supplementation or therapy for endometriosis in humans is not yet well established. Therefore, the evidence is currently rated as 2 out of 5, reflecting promising but preliminary scientific support rather than established clinical validation.
