Caryophyllene is a natural bicyclic sesquiterpene found in many essential oils, including clove oil and cannabis. Its use in supporting or treating endometriosis is primarily justified by recent scientific research, rather than traditional medicine. Caryophyllene is known to act as a selective agonist of the cannabinoid receptor type 2 (CB2), which is involved in modulating inflammation and immune responses. Preclinical studies, particularly in animal models, have shown that β-caryophyllene can reduce inflammation and pain, both of which are central to the pathology of endometriosis.

For example, studies such as "β-Caryophyllene, a CB2 receptor agonist produces multiple anti-inflammatory effects in a mouse model of endometriosis" (2014, PLoS One) have demonstrated reduced lesion size and decreased inflammatory cytokine production in mice treated with caryophyllene. These findings suggest a potential mechanism for symptom relief in endometriosis through modulation of the endocannabinoid system and suppression of pro-inflammatory mediators.

However, there is currently a lack of large-scale human clinical trials confirming its efficacy and safety for endometriosis. Thus, while there is promising scientific evidence at the preclinical level (animal and cell studies), the overall evidence rating remains low to moderate (2/5), pending further clinical investigation. There is no notable tradition of caryophyllene use for endometriosis in herbal or folk medicine.
