Sophora flavescens (also known as Ku Shen) is a traditional Chinese medicinal herb that has been historically used to address a range of respiratory conditions, including cough and asthma. Its application for emphysema, however, is primarily rooted in traditional use rather than robust scientific validation. In traditional Chinese medicine (TCM), Sophora flavescens is believed to clear heat and dry dampness, which can be interpreted as reducing inflammation and phlegm in respiratory conditions. Some classical TCM formulations containing Ku Shen are used to manage symptoms such as chronic cough and wheezing, which may overlap with emphysema symptoms.

Modern pharmacological studies have identified that Sophora flavescens contains alkaloids (notably matrine and oxymatrine) that exhibit anti-inflammatory and antioxidant properties. A few in vitro and animal studies suggest these compounds might help reduce lung inflammation and fibrosis, offering a theoretical basis for its use in chronic pulmonary diseases. However, direct clinical evidence in humans specifically demonstrating efficacy for emphysema is lacking. No large-scale randomized controlled trials have evaluated its effectiveness or safety in emphysema patients. Therefore, while there are traditional and theoretical scientific reasons for its use, the evidence base remains weak and primarily historical.
