Specialized pro-resolving mediators (SPMs) are bioactive lipid compounds derived from omega-3 fatty acids, including resolvins, protectins, and maresins. SPMs are not traditional remedies for edema, but have emerged from recent scientific research exploring their role in inflammation resolution. Edema frequently results from inflammatory processes that increase vascular permeability and fluid accumulation in tissues. SPMs have been shown in preclinical models to actively orchestrate the resolution of inflammation by limiting neutrophil infiltration, enhancing macrophage-mediated clearance of cellular debris, and restoring tissue homeostasis. Some animal studies indicate that SPMs can reduce inflammatory edema in models such as carrageenan-induced paw swelling, suggesting potential utility in controlling edema that is driven by acute inflammation. However, direct clinical evidence in humans for the use of SPMs specifically to treat or support edema is limited. Most human studies of SPMs focus on broader anti-inflammatory and pro-resolving effects, rather than explicitly measuring edema reduction as a primary outcome. As such, while the mechanistic rationale and preclinical data are promising, the current scientific evidence supporting SPMs for edema in humans is weak and largely extrapolated from animal or in vitro data. More robust clinical trials are needed to clarify efficacy and optimal usage for this indication.
