Serrapeptase is a proteolytic enzyme originally isolated from the bacterium Serratia marcescens found in the intestines of silkworms. It has been used in some countries, especially in parts of Europe and Asia, as a therapeutic agent for conditions involving inflammation and edema, particularly in the context of post-surgical or traumatic swelling. Scientific studies supporting its use are limited but do exist. Several small randomized controlled trials from the 1970s to 1990s (for example, studies on post-operative swelling following dental and ear surgeries) have shown that serrapeptase may help reduce swelling and facilitate the resorption of edema more rapidly than placebo or no treatment. The proposed mechanism is the enzyme's ability to break down inflammatory mediators and fibrin, potentially aiding fluid drainage from inflamed tissues.
However, these studies are typically small, sometimes lack rigorous design, and have not been widely replicated in recent, large-scale research. Systematic reviews have noted the paucity of high-quality evidence and called for more robust clinical trials. Regulatory agencies in many countries have not approved serrapeptase for medical use, citing insufficient evidence regarding both efficacy and safety. In summary, while there is limited scientific evidence suggesting a possible benefit of serrapeptase in treating edema, especially post-surgically, the overall quality and quantity of this evidence are low.
