Rutosides (also known as rutin) are a group of flavonoids commonly found in various plants and frequently used in Europe as venoactive drugs. There is scientific evidence supporting their use in the management of edema, particularly chronic venous insufficiency (CVI) and associated lower limb edema. Several randomized controlled trials and systematic reviews have examined the efficacy of rutosides and related flavonoids, such as hydroxyethylrutosides, in reducing leg swelling, improving symptoms like heaviness and pain, and decreasing capillary permeability in patients with CVI.

A 2016 Cochrane review (“Horse chestnut seed extract for chronic venous insufficiency”) and a 2014 review in Phytomedicine both cite studies in which rutosides demonstrated moderate effectiveness in reducing edema compared to placebo, though the quality of evidence varies and sample sizes are sometimes limited. The presumed mechanism involves the strengthening of capillary walls and reduction of vascular permeability, leading to decreased fluid leakage into interstitial spaces. Adverse effects are generally mild, and rutosides are considered safe for short-term use.

However, the evidence, while positive, is not uniformly strong—some studies show only modest benefits and methodological quality varies. Thus, rutosides are widely used in Europe and are recommended by some guidelines for CVI-related edema, but they are not universally accepted as a first-line therapy. Ongoing research may further clarify their role, but current evidence supports their use with moderate confidence.
