Proteolytic enzymes (proteases), such as bromelain, trypsin, and serratiopeptidase, have been studied for their ability to support the treatment of edema, particularly post-traumatic and post-surgical edema. The proposed mechanism involves the enzymes' ability to break down fibrin and other protein-based compounds at sites of inflammation, thus enhancing microcirculation and reducing fluid accumulation. Several randomized controlled trials and systematic reviews (mainly from Europe and Asia) suggest that oral or topical administration of proteolytic enzymes can modestly reduce edema and improve recovery times after injuries or surgeries compared to placebo or standard care. However, the quality of evidence is moderate at best, with studies often being small, heterogeneous in design, and sometimes funded by manufacturers. The most robust evidence exists for bromelain in the context of dental surgery and for serratiopeptidase in soft tissue injuries. Major medical guidelines do not currently recommend proteolytic enzymes as standard therapy for edema, primarily due to inconsistent efficacy data and concerns about methodological rigor in existing research. Thus, while there is scientific investigation and some positive findings, the overall evidence is not strong enough for broad clinical endorsement.
