Proteolytic enzymes, such as bromelain, trypsin, and chymotrypsin, have been traditionally used in some countries, particularly in parts of Europe and Asia, as adjuncts to treat edema, especially post-traumatic or post-surgical swelling. The rationale stems from their purported ability to break down fibrin and other proteinaceous debris at sites of injury, theoretically improving microcirculation and aiding in the resolution of inflammation and fluid accumulation. Clinical studies on the effectiveness of proteolytic enzymes for edema have yielded mixed results. Some small trials, mainly from the mid-20th century, suggested that oral or topical administration of these enzymes could reduce swelling and speed recovery after injuries or surgery. However, these studies often suffered from methodological limitations, such as small sample sizes, lack of proper controls, and potential publication bias. More recent systematic reviews and meta-analyses have concluded that while there may be a modest benefit, the quality of evidence is low and not sufficient to support widespread use. Despite this, proteolytic enzyme supplements are still marketed for edema on the basis of tradition and historical use, rather than robust scientific validation. Thus, the use of unspecified proteolytic enzymes for edema remains primarily tradition-based, with limited and low-quality evidence supporting efficacy.
