Proanthocyanidins, a class of polyphenolic compounds found in plants such as grape seeds and pine bark, have been investigated for their vascular and anti-inflammatory properties. Scientific studies, particularly in Europe, have explored their use in the management of chronic venous insufficiency (CVI), a condition that can lead to edema (swelling) in the lower limbs. Clinical trials have suggested that standardized extracts containing proanthocyanidins, such as French maritime pine bark extract (Pycnogenol) and grape seed extract, can reduce lower limb swelling, improve microcirculation, and decrease symptoms associated with CVI, including edema. The proposed mechanisms include strengthening capillary walls, reducing capillary permeability, and exerting antioxidant effects that protect vascular tissues. Systematic reviews and meta-analyses (e.g., Liu et al., 2017, Phytomedicine; Ghosh & Scheepens, 2009, Nutrition & Metabolism) highlight modest but statistically significant benefits for edema reduction in CVI patients. However, the quality of studies varies, with some limitations in sample size and methodological rigor. The evidence does not strongly support use for all types of edema (such as those caused by heart, liver, or kidney disease), but for venous-origin edema, particularly in chronic venous insufficiency, there is moderate scientific support. Therefore, proanthocyanidins are justified for use in supporting venous health and managing edema related to CVI, though more high-quality, large-scale trials are warranted.
