Ligustilide is a major bioactive compound found in traditional Chinese medicinal herbs, such as Angelica sinensis (dang gui) and Ligusticum chuanxiong (chuanxiong). These herbs have been traditionally used in Chinese medicine for the treatment of various ailments, including circulatory disorders and edema (fluid accumulation). However, the use of ligustilide specifically for edema is based primarily on traditional practices rather than robust scientific validation. Historical Chinese medical texts often attribute the reduction of swelling and improvement of blood flow to herbal formulations containing these plants, which are thought to "invigorate blood" and "dispel wind and dampness."

From a scientific perspective, some preclinical studies indicate that ligustilide may have anti-inflammatory, vasodilatory, and neuroprotective effects. These actions could theoretically influence factors involved in edema, such as vascular permeability and inflammation. Nonetheless, direct clinical or experimental evidence specifically demonstrating ligustilide’s efficacy against edema is very limited. No large-scale human trials or well-established animal studies have confirmed its effectiveness for this indication. Therefore, the use of ligustilide for edema remains primarily rooted in traditional herbal practice, with only limited preliminary scientific rationale and a lack of high-quality, condition-specific evidence.
