Flavanols, a subclass of flavonoids found in foods such as cocoa, tea, grapes, and apples, have been studied for their potential vascular and anti-inflammatory effects. Their use in supporting or treating edema (the accumulation of fluid in tissues) is primarily based on their impact on vascular function, capillary permeability, and inflammation. Several clinical studies and reviews have focused on flavanol-rich extracts, particularly those from grape seeds (Vitis vinifera) and pine bark (Pinus pinaster), which are often sold as “flavanol mixtures.” These have demonstrated modest efficacy in chronic venous insufficiency (CVI), a condition that frequently causes leg edema. Flavanols appear to strengthen capillary walls, reduce capillary leakage, improve venous tone, and have antioxidant properties that may address underlying vascular inflammation.
Meta-analyses and systematic reviews (e.g., Cochrane 2016) have concluded that proanthocyanidin-rich flavanol extracts can reduce leg swelling and symptoms in CVI patients, though the overall quality of evidence is moderate and studies are often short-term. The European Medicines Agency (EMA) recognizes certain flavanol-rich preparations as “traditional herbal medicinal products” for symptoms of CVI, including edema. However, these effects are modest, and more robust, long-term trials are needed. In summary, there is scientific evidence supporting the use of flavanol mixtures for edema secondary to venous insufficiency, but the evidence is not strong enough to recommend for all types of edema.
