Escin, a mixture of triterpene saponins derived from horse chestnut (Aesculus hippocastanum), is scientifically validated for use in the management of edema, particularly chronic venous insufficiency (CVI) and related swelling. Multiple randomized, double-blind, placebo-controlled clinical trials have demonstrated that oral or topical escin can significantly reduce leg volume, ankle circumference, and other edema-related symptoms in patients with CVI compared to placebo. Its mechanism involves strengthening capillary walls, reducing capillary permeability, and exerting anti-inflammatory effects by inhibiting enzymes that degrade proteoglycans in vessel walls. Systematic reviews and meta-analyses, including a 2012 Cochrane review, have concluded that horse chestnut seed extract (standardized to escin) is more effective than placebo and comparable in efficacy to reference medications such as compression therapy in the short term. Side effects are relatively mild and infrequent, though not absent. While some traditional use exists, the primary justification for escin’s use in edema is robust scientific evidence, especially for chronic venous disorders. However, its efficacy for other causes of edema (e.g., heart failure, renal disease) is not well established.
