Enzyme therapy, particularly the use of proteolytic (protein-digesting) enzymes such as bromelain, trypsin, chymotrypsin, and serrapeptase, has some scientific support for the management of edema, especially post-traumatic and post-surgical edema. Several clinical studies have demonstrated that these enzymes can help reduce inflammation and promote the breakdown of fibrin and other inflammatory mediators, which can contribute to swelling. For instance, a number of randomized controlled trials and meta-analyses have found that oral proteolytic enzymes can significantly reduce the duration and severity of edema following injuries or surgical procedures compared to placebo. The proposed mechanisms include enhanced degradation of plasma proteins in inflamed tissues, improved microcirculation, and modulation of inflammatory responses.
However, while there is moderate evidence supporting the use of certain enzymes in specific types of edema (mainly post-injury or surgery), the evidence is less robust for generalized or chronic edema, such as that seen in heart failure or kidney disease. Most studies focus on acute, localized swelling rather than systemic fluid accumulation. Additionally, the quality and size of clinical trials vary, and enzyme preparations are heterogeneous. Thus, while there is scientific validation for the use of some enzymes in specific contexts, more large-scale, high-quality trials are needed to fully establish their efficacy and safety for broader indications of edema.
