Bioflavonoids (also known as flavonoids) are plant-derived compounds commonly found in fruits and vegetables. Their use for edema primarily stems from traditional and historical practices, especially in herbal and alternative medicine systems in Europe and Asia. The rationale for their use is based on their purported ability to strengthen capillary walls, enhance microcirculation, and reduce capillary permeability, which could theoretically decrease fluid leakage and swelling (edema). Notably, preparations containing diosmin and hesperidin (two specific bioflavonoids) have been incorporated in some European pharmacopoeias for the treatment of chronic venous insufficiency and associated symptoms such as edema. However, scientific evidence supporting the efficacy of mixed bioflavonoids for edema is limited and generally of low quality. Most clinical research has focused on specific, purified flavonoids (e.g., diosmin, rutin) rather than generic mixtures. Some small studies have shown modest benefits for venous insufficiency and related edema when using certain flavonoid formulations, but these findings are not robust or widely generalizable. Systematic reviews and meta-analyses often conclude that while flavonoids may have a mild effect on symptoms like leg swelling and heaviness, the quality of evidence is low, and more rigorous research is needed. In summary, the use of bioflavonoid mixtures for edema is primarily based on traditional use and theoretical mechanisms, with limited and low-quality scientific support to date.
