Albumin is a plasma protein that plays a crucial role in maintaining oncotic pressure, which helps keep fluid within blood vessels. Its use in treating edema—particularly in the context of hypoalbuminemia (low serum albumin)—is supported by scientific rationale and clinical evidence, though the strength of evidence varies by underlying cause.

In conditions such as nephrotic syndrome, liver cirrhosis with ascites, and critical illness where hypoalbuminemia contributes to fluid leakage into tissues, intravenous albumin administration can temporarily raise plasma oncotic pressure and facilitate mobilization of interstitial fluid back into the vascular compartment. This is especially noted when combined with diuretics (as in nephrotic syndrome) or with paracentesis (as in liver cirrhosis with large-volume paracentesis).

However, large randomized controlled trials have not consistently shown long-term mortality or morbidity benefit for albumin use in general edema or critical illness, and its routine use is not recommended outside of specific indications. Guidelines suggest albumin in select settings, such as large-volume paracentesis, but not for generalized edema without underlying hypoalbuminemia. Overuse is discouraged due to cost and limited impact on clinical outcomes in many contexts.
