Omega-3 fatty acids, particularly eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) found in fish oil, have been studied for their potential to alleviate dysmenorrhea (painful menstruation). Scientific interest is based on the anti-inflammatory properties of omega-3s. Prostaglandins, hormone-like compounds responsible for uterine contractions and menstrual pain, are derived from fatty acids. Omega-3s may decrease the synthesis of pro-inflammatory prostaglandins (PGE2, PGF2α) and increase anti-inflammatory prostaglandins, potentially reducing menstrual pain and cramping.
Several randomized controlled trials (RCTs) and systematic reviews support the use of omega-3s for dysmenorrhea. For example, a 2012 RCT published in the Caspi Journal of Reproductive Medicine found that women who received 1,000 mg/day of omega-3 supplements for three months experienced significantly reduced pain intensity compared to placebo. Another meta-analysis (2020, Journal of Dietary Supplements) concluded that omega-3 supplementation could significantly decrease pain severity in women with primary dysmenorrhea. The effect is generally modest, and some studies have methodological limitations, such as small sample sizes or short duration.
While more extensive and longer-term studies are needed, current evidence suggests that omega-3 supplementation is a reasonable adjunctive therapy for dysmenorrhea, especially for women seeking non-pharmacological options. Its safety profile is favorable, with few side effects reported at commonly used doses.
