Paeonia officinalis (European peony) has a long history of use in traditional European herbal medicine, particularly for women's health issues such as dysmenorrhea (painful menstruation). Historical texts from the Middle Ages and early modern period reference peony root as a common remedy for menstrual cramps and other gynecological complaints. The root was typically used in the form of decoctions, tinctures, or powders. The rationale in traditional herbalism is based on the presumed antispasmodic and calming effects of the plant, which were thought to help relieve uterine muscle spasms and ease menstrual pain.

However, while there are some modern pharmacological studies on Paeonia species (notably Paeonia lactiflora, the Chinese peony), there is little direct scientific research specifically on European peony (Paeonia officinalis) and its effects on dysmenorrhea. Most of the scientific interest and evidence focuses on the Chinese peony, which is a different species and used in Traditional Chinese Medicine. As such, the use of European peony for dysmenorrhea is primarily justified by tradition rather than robust scientific evidence, and modern clinical trials specific to Paeonia officinalis and this indication are lacking.
