Boswellia serrata, commonly known as Indian frankincense, has a long history in traditional medicine for its anti-inflammatory properties. In recent years, its use for dysmenorrhea (painful menstruation) has gained some scientific interest. A few clinical studies have investigated Boswellia's effectiveness in reducing menstrual pain and related symptoms. The anti-inflammatory action is primarily attributed to boswellic acids, which inhibit pro-inflammatory enzymes such as 5-lipoxygenase.

A notable randomized controlled trial published in 2019 evaluated the effect of Boswellia serrata extract (in combination with ginger) on primary dysmenorrhea. The study found a statistically significant reduction in pain severity compared to placebo. Another small pilot study suggested that Boswellia, either alone or as part of herbal formulations, can moderately reduce pain and improve quality of life in women with dysmenorrhea. However, these studies often include Boswellia as one component of a multi-herbal preparation, making it difficult to attribute effects solely to Boswellia.

Systematic reviews and meta-analyses acknowledge the potential but highlight the need for larger, well-designed trials to confirm efficacy and establish dosage guidelines. While the evidence base is not yet robust, current research suggests that Boswellia may offer some benefit for dysmenorrhea through its anti-inflammatory effects.
