Bisabolol, a naturally occurring monocyclic sesquiterpene alcohol found in chamomile (Matricaria recutita), is widely used in skincare formulations, including those targeting diaper rash. Its primary scientific value lies in its anti-inflammatory, soothing, and antimicrobial properties. A number of in vitro and in vivo studies have demonstrated that bisabolol can inhibit pro-inflammatory cytokines and reduce erythema, making it a logical ingredient for products intended to calm irritated skin. Clinical studies specifically addressing bisabolol’s effect on diaper rash are limited; however, its efficacy in alleviating symptoms of mild dermatitis and irritation provides a reasonable basis for its inclusion. Traditional herbal remedies have utilized chamomile preparations for skin soothing, and bisabolol is recognized as a key active component responsible for these effects. In modern formulations, bisabolol is favored for its low allergenic potential and ability to enhance the penetration of other active ingredients. Despite these promising mechanisms and its successful use in products intended for sensitive skin, direct, high-quality clinical evidence supporting bisabolol’s effectiveness specifically for diaper rash remains limited. Most support comes from extrapolated data on its general effects on skin inflammation and irritation. Therefore, while its use is scientifically plausible and supported by mechanistic studies, the direct evidence for diaper rash is moderate to low, warranting a rating of 2 out of 5.
