Aloe vera has been used traditionally for various skin conditions, including burns, wounds, and irritations, due to its soothing, anti-inflammatory, and moisturizing properties. In the context of diaper rash, there is some scientific evidence supporting its use, though the overall quality and quantity of studies are limited. A small number of clinical trials and observational studies suggest that topical application of Aloe vera gel may help reduce erythema (redness), irritation, and promote healing of mild to moderate diaper rash. Its mechanism is believed to involve anti-inflammatory compounds, polysaccharides that aid in skin repair, and moisturizing effects that can help restore the skin barrier. A randomized controlled trial published in the "Iranian Journal of Pediatrics" (2012) compared Aloe vera cream with calendula ointment for the treatment of diaper dermatitis and found both to be effective, with no significant difference between the two. Another study in "Pediatric Dermatology" (2014) included Aloe vera as part of a combination treatment that was effective in reducing diaper rash severity. However, these studies are relatively small and further high-quality, large-scale research is needed to establish definitive efficacy and safety. In summary, there is modest scientific support for the use of Aloe vera in diaper rash, primarily based on its anti-inflammatory and skin-soothing properties, but the evidence base is not strong. It is generally regarded as safe for topical use in infants, but care should be taken to use pure formulations without added fragrances or potential irritants.
