Verbascoside, a phenylpropanoid glycoside found in various medicinal plants (such as Verbascum and Plantago species), has attracted interest for its antioxidant, anti-inflammatory, and neuroprotective properties. Although verbascoside is not a standard treatment for diabetic retinopathy (DR), several preclinical studies suggest a scientific rationale for its supportive use. In animal models of diabetes and retinal injury, verbascoside administration has been shown to reduce oxidative stress, inhibit inflammation, and decrease apoptosis in retinal cells—mechanisms implicated in the pathogenesis of DR. For example, a 2019 study in diabetic rats demonstrated that verbascoside treatment attenuated retinal vascular leakage and reduced levels of inflammatory cytokines and reactive oxygen species. In vitro studies on retinal pigment epithelial cells have also reported protective effects against high-glucose-induced damage. However, there are currently no published human clinical trials investigating verbascoside specifically for diabetic retinopathy. The available evidence is limited to laboratory and animal research, making the translation to clinical recommendations premature. While the mechanistic data are promising and justify further investigation, the current scientific evidence is preliminary and insufficient for routine clinical use in DR. Thus, verbascoside's use for diabetic retinopathy is supported by early scientific (preclinical) evidence, but lacks robust clinical validation.
