Osthole is a natural coumarin derivative isolated from plants such as Cnidium monnieri and has been investigated for various pharmacological effects, including potential benefits in diabetic complications. There is emerging scientific evidence, primarily from preclinical studies, suggesting that osthole may have protective effects against diabetic retinopathy (DR). For example, in a 2021 study (PMID: 33840911), osthole was shown to attenuate retinal vascular leakage and inhibit inflammation and oxidative stress in the retinas of diabetic rats. The proposed mechanisms include suppression of inflammatory cytokines, reduction of vascular endothelial growth factor (VEGF) expression, and inhibition of oxidative damage—all factors implicated in the pathogenesis of DR.

However, it is important to note that, so far, the evidence supporting osthole’s use for diabetic retinopathy is limited to animal and cellular models. There are no published human clinical trials directly assessing the efficacy or safety of osthole for this indication. Therefore, while there is a scientific rationale and preliminary validation, the evidence is still in the early stages and not robust enough for clinical recommendation. Further research, especially well-designed human studies, is needed to determine its potential therapeutic role in diabetic retinopathy.
