Flavan-3-ols, a class of polyphenolic compounds found abundantly in foods such as tea, cocoa, apples, and berries, have been investigated for their potential benefits in diabetic retinopathy (DR). The scientific basis for their use primarily stems from preclinical evidence. Laboratory studies and animal models suggest that flavan-3-ols possess antioxidant and anti-inflammatory properties. These properties may help mitigate the oxidative stress and inflammatory processes implicated in the progression of DR. Additionally, some studies have shown that flavan-3-ols can improve endothelial function and microvascular health, which are relevant to the pathology of diabetic retinopathy. However, human clinical trials specifically evaluating flavan-3-ols for the prevention or treatment of diabetic retinopathy are limited and generally lack robust, high-quality data. A few small studies have reported improvements in biomarkers related to vascular health in diabetic patients, but direct evidence of benefit in DR is sparse. No major clinical guidelines currently recommend flavan-3-ols as a treatment for diabetic retinopathy. In summary, while there is a scientific rationale and encouraging preclinical data, the clinical evidence is insufficient to strongly support the use of flavan-3-ols for diabetic retinopathy at this time. More rigorous human studies are needed to confirm efficacy and safety in this specific context.
