Caffeic acid, a polyphenolic compound found in many plants, has been investigated for its potential therapeutic effects in diabetic retinopathy, but the evidence is primarily preclinical. Studies in cell cultures and animal models have shown that caffeic acid can exert antioxidant and anti-inflammatory effects, both of which are relevant to the pathogenesis of diabetic retinopathy. For example, research published in the journal Life Sciences (2019) demonstrated that caffeic acid reduced oxidative stress and apoptosis in retinal tissues of diabetic rats, suggesting a protective effect. Other studies have shown that caffeic acid may inhibit pathways involved in vascular leakage and retinal neovascularization, key processes in diabetic retinopathy progression (Journal of Ocular Pharmacology and Therapeutics, 2017). However, there are currently no robust clinical trials in humans evaluating the efficacy or safety of caffeic acid supplementation specifically for diabetic retinopathy. Most of the available evidence is limited to laboratory and animal studies, and clinical relevance remains uncertain. Therefore, while there is scientific rationale and preliminary evidence supporting its potential benefit, its use for diabetic retinopathy is not yet validated in human medicine. Further research, especially well-designed human clinical trials, is needed before caffeic acid can be recommended for this indication.
