Alpha-terpineol is a naturally occurring monoterpene alcohol found in the essential oils of several plants, including pine and tea tree. Its use in supporting or treating dermatitis is primarily based on its demonstrated anti-inflammatory and antimicrobial properties in preclinical studies. Several in vitro and animal studies have shown that alpha-terpineol can inhibit the production of pro-inflammatory cytokines, such as TNF-α and IL-6, which are involved in the pathogenesis of inflammatory skin conditions like dermatitis. Additionally, alpha-terpineol has shown activity against various microbial pathogens that may complicate dermatitis, including Staphylococcus aureus.

However, while these findings suggest a plausible mechanistic rationale, direct clinical evidence in humans with dermatitis is currently lacking. Most published research is limited to laboratory or animal models, and there are no robust randomized controlled trials evaluating the efficacy of alpha-terpineol as a treatment for dermatitis in humans. Some topical formulations containing alpha-terpineol or related essential oils have been used traditionally for skin soothing, but scientific validation remains preliminary. Thus, while there is scientific interest and early evidence, the overall strength of evidence for alpha-terpineol’s effectiveness in dermatitis is low (rated 2 out of 5), and further clinical studies are needed.
