Vitamin B12 (cobalamin) is an essential water-soluble vitamin involved in neurological function and the synthesis of neurotransmitters. There is some scientific rationale for its use in supporting or treating depression, primarily in individuals who are deficient in B12. Several observational studies have observed an association between low serum B12 levels and increased risk or severity of depressive symptoms. Randomized controlled trials (RCTs) on supplementation show only modest or inconsistent benefits, mostly in populations with low baseline B12 or in combination with antidepressant medication. A 2013 systematic review (Almeida et al., 2013, Am J Geriatr Psychiatry) concluded that while low B12 is associated with depression, supplementation does not consistently improve mood in non-deficient individuals. A 2017 meta-analysis (Syed et al., 2017, Psychopharmacology) found some evidence that B-vitamin supplementation may reduce depressive symptoms, but mainly in people with existing deficiencies or in adjunct to antidepressant therapy. Overall, the evidence supports B12 supplementation for individuals who are deficient, but not as a primary treatment for depression in the general population. There is no strong evidence for benefit in people with normal B12 status.
