5-Methyltetrahydrofolate (5-MTHF) is the bioactive form of folate, and the glucosamine salt is a stable supplement form. Scientific evidence suggests that folate, particularly 5-MTHF, may play a supportive role in the treatment of depression, especially for individuals with low folate status or specific genetic polymorphisms (such as MTHFR C677T) that impair folate metabolism. Several clinical trials and meta-analyses indicate that folate supplementation, including 5-MTHF, can modestly enhance the effectiveness of standard antidepressant therapies, particularly in cases of treatment-resistant depression. 5-MTHF is involved in the one-carbon cycle and methylation processes essential for neurotransmitter synthesis (including serotonin, dopamine, and norepinephrine). Low folate levels have been associated with a higher risk of depression, and correcting this deficiency can improve mood in some individuals. A 2012 meta-analysis (Papakostas et al., Am J Psychiatry) found that adjunctive use of L-methylfolate (5-MTHF) led to greater improvements in depressive symptoms in patients who did not respond adequately to selective serotonin reuptake inhibitors (SSRIs). However, while there is evidence of benefit, not all studies have shown positive results, and larger, well-designed trials are still needed. The evidence base is stronger for individuals with folate deficiency or genetic folate metabolism issues, and weaker for the general population. Thus, 5-MTHF is often considered as an adjunctive therapy rather than a primary treatment for depression.
