Vitamin E, specifically alpha-tocopheryl polyethylene glycol succinate (TPGS), is a water-soluble derivative of vitamin E frequently used as a drug delivery enhancer rather than as a direct therapeutic agent. However, vitamin E in general (including other forms like alpha-tocopherol) has been studied for its antioxidant properties and potential role in neuroprotection, with some research focusing on dementia and Alzheimer’s disease. The rationale for its use is based on the hypothesis that oxidative stress contributes to neurodegenerative diseases, and antioxidants like vitamin E may help mitigate neuronal damage.

Some clinical trials (e.g., the 1997 Sano et al. study in the New England Journal of Medicine) showed that high-dose vitamin E (2000 IU/day) modestly delayed functional decline in patients with moderate Alzheimer’s disease. However, subsequent meta-analyses and trials have yielded mixed results, with concerns about efficacy and potential adverse effects at high doses (e.g., increased risk of hemorrhagic stroke or all-cause mortality).
