Urolithin A is a gut microbiome-derived metabolite from dietary ellagitannins (found in pomegranates, berries, and nuts). Its use for dementia is justified primarily by emerging scientific evidence, rather than tradition, as there is no historical precedent for its use in traditional medicine for cognitive disorders. Preclinical studies indicate that Urolithin A may exert neuroprotective effects relevant to dementia, particularly Alzheimer’s disease. Its mechanisms include enhancement of mitochondrial function via mitophagy, reduction of neuroinflammation, and protection against amyloid-beta and tau pathology in animal and cellular models (Fang et al., 2021). Specifically, Urolithin A has been shown to improve mitochondrial health in neurons, which is a key factor in age-related cognitive decline.

However, current evidence is limited primarily to laboratory and animal research. Human studies examining cognitive outcomes are scarce, with some early-phase trials focusing on mitochondrial health and muscle function rather than direct cognitive endpoints. While the mechanistic rationale is strong, and initial findings are promising, there is insufficient clinical trial data to conclusively recommend Urolithin A for dementia treatment or prevention.
