Homotaurine, also known as tramiprosate, is a compound originally derived from seaweed and structurally similar to the neurotransmitter GABA. It has been investigated as a potential treatment for Alzheimer's disease and related dementias due to its purported ability to inhibit the aggregation of beta-amyloid proteins, a hallmark of Alzheimer's pathology. Several clinical trials, including large phase III studies (e.g., the Alphase study), have evaluated homotaurine in patients with mild to moderate Alzheimer's disease. The results showed that while homotaurine was generally safe and well-tolerated, its efficacy in improving cognitive outcomes was limited and did not meet primary endpoints for slowing cognitive decline in the overall study populations. However, some post hoc analyses suggested possible benefit in specific subgroups, such as individuals without the APOE4 allele. Despite these findings, regulatory agencies have not approved homotaurine as a pharmaceutical treatment for dementia or Alzheimer's disease. It is sometimes marketed as a dietary supplement in some countries for "cognitive support," but the scientific evidence supporting its effectiveness remains weak to modest at best.
